Travel Document

A Filipino national may apply for a Travel Document (TD) at the Consulate in lieu of an expired or lost Philippine passport if said national has an urgent travel to the Philippines. The following may be considered as urgent travel:

  1. If the applicant is returning to the Philippines due to the death or serious illness of an immediate member of the family (spouse, children, parents and siblings ONLY); or
  2. If the applicant for TD is for deportation or repatriation.

In view of the above, an applicant for TD is required to submit a valid proof of urgent travel.

Reminders:

  1. A travel document may only be used for emergency or urgent travels and cannot be conveniently used to avoid going through the passport application/renewal process.
  2. A travel document must be used for one-way, direct travel to the Philippines (no connecting flights, no layovers) within 30 days from the date of issuance.
  3. A TD is valid for one (1) month only from the date of issuance.
  4. This service is NOT for the issuance of passport nor for notarization of Parental Travel Permits

Requirements

Applicants may send their request for issuance of a TD by emailing passport@philcongen-toronto.com. Walk-in applicants for Travel Documents may also be accommodated during business days between 9 a.m. and 12 p.m.

The following are the requirements for TD:

  1. Duly filled-out application form: Travel Document Application Form
  2. Four (4) passport-size photographs (4.5cm x 3.5cm, full front view with white background);
  3. Original and photocopy of Philippine passport. For lost passports, copy of Philippine passport, if available, with duly filled-out Affidavit of Loss: Affidavit of Explanation on the Loss of Passport
  4. Original and copy of proof of Immigration status in Canada i.e., Dual Citizenship Certificate, Permanent Residence Card, Work Permit, Visitor Permit or copy of tourist visa;
  5. Copy of travel itinerary or airline tickets, with the most direct flight (no layovers, no connecting flights) from Canada to Philippines;
  6. Fee of CAD$43.50 payable in cash, credit/debit card, bank draft or postal money order payable to the Philippine Consulate General; and
  7. Documents showing proof of urgent travel e.g. Death Certificate, Medical Certificate, Deportation/Exclusion Order (if applicable), etc.

Approximate Processing Period

Processing at the Consulate takes one (1) business day upon receipt of the complete documents. Please be reminded that the validity of TD is one (1) month only from the date of issue. NOTE: (1) A travel document may be applied only for emergency reasons or urgent cases and should not be used for convenience in order to avoid going through the passport renewal process; (2) A Travel Document is valid for a maximum of 30 days from date of issue and must be used for direct travel to the Philippines.
