Report of Marriage, Birth & Death

BIRTH
For recording of births abroad of children born to Filipino parent(s), the following are required:
  1. Personal Appearance (required if a new passport is applied)
  2. Original and three (3) Photocopies of Statement of Live birth (long form) issued by the office of the Civil Registrar General, Canada
  3. Duly Accomplished and Notarized Report of Birth Forms (in four Copies) Fee: C$33.75
  4. 4 Copies of Each of the following documentary evidence of Philippine Citizenship of Father and Mother at the time of birth of child:
    1. Marriage Contract of Parents (if married)
    2. birth certificate
    3. Philippine Passport
    4. Certificate of Naturalization
  5. Four (4) Passport - size photographs
  6. Affidavit of illegitimacy (if not married) Fee: C$33.75
  7. Four copies of Affidavit of Delayed Registration if registration took place one (1) year after birth. Fee: C$33.75
  8. If child is also applying for a passport, please provide an extra copy of both the statement of live birth and the Report of Birth
DEATH
For recording deaths of Filipino nationals abroad, the following are required: (Excluding former Filipinos who died without reacquiring Philippine Citizenship)
  • Copy of Death Certificate;
  • Passport of the deceased;
  • A duly accomplished and notarized form (Report of Death of Philippine Citizen) available at the Consulate General
  • Fee of CAD$33.75
SHIPMENT OF HUMAN REMAINS/ CREMATED REMAINS IN URN: For shipment of human remains to the Philippines, a consular mortuary certificate must be issued by the Philippine Consulate General in Toronto. The following should be submitted to the Consulate in two (2) sets of original documents* and five (5) sets of photocopies of the orig. documents:
  • Passport of the deceased
  • Statement of Death (Form 15)*
  • Medical Certificate of Death (Form 16)*
  • Embalmer’s certificate (under oath before a Notary Public or person authorized to administer oath and include a declaration that the casket contains only the remains of the deceased
  • Certification of Medical Health Officer*
  • Certificate/ Permit for shipment outside of Ontario
  • Certifications of the Board of Funeral Services, and
  • Flight itinerary of the shipment and information on the consignee & airway bill number
For shipment of cremated remains properly contained in an urn (must not be a metal urn as prohibited by Canadian authorities), following are the documents required:
  • Passport of the deceased
  • Statement of Death (Form 15)*
  • Medical Certificate of Death (Form 16)*
  • Certificate of Cremation*
  • Passport of person carrying the urn; and
  • Flight itinerary of the person carrying the urn.
If the deceased is a Philippine passport holder, fee is C$67.50. If Canadian, C$33.75.

MARRIAGE
For reporting of marriage solemnized in Canada
  • Duly accomplished and notarized Report of Marriage (ROM) Forms (in four original copies).
  • Original and three (3) photocopies of Marriage License or Registration of Marriage (long form) issued by the Office of the Civil Registrar General;
  • If applicant had a previous marriage, certificate of Finality of Nullity of Marriage issued in the Philippines must be authenticated by the Department of Foreign Affairs( in four copies);
  • Four (4) 2x2 photographs each of husband and wife
  • Four (4) photocopies of the data page of the passport of the couple
PLEASE BE INFORMED THAT, FOR PURPOSES OF RENEWING A PASSPORT OF A MARRIED FEMALE APPLICANT REFLECTING THE SURNAME OF THE HUSBAND, A DULY-AUTHENTICATED COPY OF THE ROM MUST BE SECURED FIRST FROM THE NSO IN THE PHILIPPINES.

THE CONSULATE PREPARES TRANSMITTAL OF ROMs TO THE NSO, THROUGH THE DFA, ON A MONTHLY BASIS.

Fee for Report of Marriage is CAD$33.75 payable in cash, money order or certified cheque payable to the Philippine Consulate General.

PLEASE NOTE THAT THE CONSULATE GENERAL IN TORONTO CAN ONLY ACCEPT REPORTS OF BIRTHS, MARRIAGES, AND DEATHS THAT OCCURRED IN AREAS UNDER ITS CONSULAR JURISDICTION - MANITOBA AND PARTS OF ONTARIO, EAST OF KINGSTON.

FOR MAILING OF DOCUMENTS, APPLICANT MUST PROVIDE A SELF-ADDRESSED/STAMPED MAILING ENVELOPE, PREFERABLY AN XPRESSPOST FROM CANADA POST FOR TRACKING PURPOSES.

Please note that the Consulate will not be responsible for documents that may be lost or damaged in the mail.

NOTE - Payment of Fees
The Consulate accepts ONLY Cash, Bank Draft, Money Orders or Certified Cheques
