Pursuant to instructions from the Department of Foreign Affairs, the Philippine Consulate General in Toronto informs the public that effective 01 January 2018, the Department will implement the use of the revised passport application form. Please note of the following revised forms:
a. New Regular Passport Application Form (for Adult, first time passport application)
b. Renewal Regular Passport Application Form (for Adult)
c. Regular Passport Application Form (for Minor)
The Consulate requests the cooperation of the public in accomplishing the appropriate form.
The Consulate also takes this opportunity to inform the public that effective 01 January 2018, the Consulate will issue regular/ordinary passports with a validity of ten (10) years for Filipino citizens who are eighteen (18) years old and above and regular/ordinary passports with a validity of five (5) years for those below eighteen years of age.
